Instructions ? Create a 1-2 page questionnaire for your patients / clients to
identify their cultural needs that might affect their healthcare ? The
questions should address cultural components such as: religion, traditions, family,
food, language, and traditional healthcare practices. Requirements ?
Questionnaire should be about 1–2 pages in length
